Waiver and Release of Liability

QB Four U

Effective Date: December 1st, 2025

This Waiver and Release of Liability (“Waiver”) is entered into by the undersigned parent or legal guardian (“Participant” or “Athlete” and “Guardian”) in favor of QB Four U, its owners, coaches, staff, contractors, volunteers, and affiliates.

1. Acknowledgment of Participation

I understand that my child (or I, if over 18) is voluntarily participating in quarterback training, flag football training, camps, clinics, private sessions, group sessions, and/or online or virtual training programs provided by QB Four U.

2. Assumption of Risk

I acknowledge that participation in football and flag football training involves inherent risks, including but not limited to:

I knowingly and voluntarily assume all risks, known and unknown, associated with participation in QB Four U programs.

3. Release of Liability

In consideration of participation, I hereby release, waive, discharge, and hold harmless QB Four U and its owners, coaches, staff, affiliates, and representatives from any and all claims, demands, causes of action, or liability arising out of or related to participation in any QB Four U program, including but not limited to personal injury, illness, or property damage, except where caused by gross negligence or willful misconduct as required by law.

4. Medical Responsibility

I certify that the athlete is physically able to participate in athletic training activities and has no medical condition that would prevent safe participation.

I understand that QB Four U does not provide medical insurance coverage. In the event of injury or illness, I accept full responsibility for medical expenses.

I authorize QB Four U staff to seek emergency medical treatment if necessary and agree to be responsible for any associated costs.

I understand that participation in QB Four U programs requires respectful behavior, discipline, and adherence to coaching instructions.

QB Four U reserves the right to remove or suspend any participant for unsafe behavior or conduct that negatively impacts the training environment, without refund.

7. Photo & Video Release

I grant QB Four U permission to photograph, record, or otherwise capture the likeness of the athlete during training sessions, events, or online programs for educational and promotional purposes, including use on websites, social media, marketing materials, and digital platforms.

This release is granted without expectation of compensation.
If I wish to opt out, I will submit a written request to QB Four U.

8. Intellectual Property (Online Content)

I understand that all training materials, videos, drills, and digital resources provided by QB Four U are proprietary and for personal use only.

I agree not to copy, share, reproduce, or distribute any content without written permission from QB Four U.

9. Governing Law

This Waiver shall be governed by and interpreted in accordance with the laws of the state in which QB Four U operates.
